How to Enroll: Class
You must pre-register for all classes/workshops
To pre-register:
1. Complete a registration form for the class/workshop (listed below)
2. Obtain a copy of your pet’s vaccination record showing proof of the required vaccinations and include it with your registration form. If you do not have a copy of your pet’s vaccinations, please call your veterinarian and ask them to fax a copy to us (Attn: Donna)
3. Deliver your registration form, vaccine records, and payment to the clinic.
- Registrations may be hand delivered, scanned & emailed, or faxed to us at (765) 463-3112.
- Please do not mail in your registration forms via US Mail.
- We accept Mastercard, Visa, Discover, personal checks, or cash. Make checks payable to Blair Animal Clinic.
Once we receive your registration and payment, you will receive a confirmation by e-mail.
